Sea Cadet/Leaguers Application Forms
You will need the following forms to apply to the Sea Cadets or Navy League Cadets program. Note that you or your child must attend a mandatory information session before turning in these forms as outlined on the How to Join link above.

You may bring these application forms with you to the information session if you would like, but do not sign them until you've spoken with the Commanding Officer. Once signed, application forms and fees are to be turned in to the Administration Officer.

Member Information and Declarations
(Pages 1 and 2)
Must also provide most recent school report card (not progress report!)

Report of Medical History
(Pages 3 and 4)
A medical self-assessment. Must also provide a copy of current immunizations record

Report of Medical Examination
(Page 5 and 6)
A medical examination similar to a high school sports physical is required for all cadet applicants to the Naval Sea Cadet Corps (NSCC) or Navy League Cadet Corps (NLCC). No one will be denied admission to the NSCC/NLCC due to a medical disability. Where a medical condition precludes full, unlimited participation, a Request for Accommodation (Page 9 and 10 below) may be presented by the parent or guardian for review so the cadet may participate in NSCC activities to the maximum extent possible

To be filled out only if cadet is on regular medications.

Request for Accommodation
(Page 9 and 10)
To be filled out if a medical condition precludes full, unlimited participation.

Parental Support Agreement
(Page 11)
To be filled out by the parent or guardian.


Adult Sea Cadet Application Forms
You will need the following forms to apply to the Adult Sea Cadets program.  Note that you must attend a mandatory information session before turning in these forms as outlined on the How to Join link above. You may bring these application forms with you to the information session if you would like, but do not sign them until you've spoken with the Commanding Officer. Once signed, application forms and fees are to be turned in to the Administration Officer.

If active duty military, must also provide a Statement of Service by current command.
For prior service applicants, must also provide a copy of your Record of Discharge (DD214).


Request for Reference
(Page 4)
Must make 3 copies of this form and have them filled out by three acquaintances.

Report of Medical History
(Page 5 and 6)
A medical self-assessment. Adult applicants must be in good health commensurate with their age group and be free from any ailment or condition that would prevent them from satisfactorily performing their primary duty of supervising youth. NOTE: Adults not physically qualified to perform all duties may still participate with a waiver from NHQ considering the parameters of their expected contribution to the NSCC program.
